Online Medical Records Request

When submitting a request online, please complete all required fields. You will need to upload a copy of your government issued photo ID.
Requests will be processed within five to ten business days of receipt. Requests are fulfilled by our medical records partner, Verisma.

You can Request Your Medical Records online, by fax or in person.
To fax your request, please complete the appropriate form below and fax to 866.538.2334.

To pick up records in person, visit the Frye Regional Medical Center Main Entrance, turn left before the registration desk, and use the phone at the desk to contact Health Information Management.
Hours of Operation:
Monday – Friday, 8:00 am – 4:30 pm.
Closed on weekends and Holidays.
